#4 changed sample.avif and added fixtures/documents/mermaid.md without touching the manifest, so scripts/verify.py failed on an unlisted fixture.

This lists mermaid.md in manifest.json and FORMAT-INDEX.md, records the new AVIF size and hash, and adds the document to generate.py so a regeneration keeps it. python3 scripts/verify.py passes (229 fixtures).
